Qualifications

Responsibilities
- Assess residents' nutrition needs and document plans in the electronic medical record.
- Develop and evaluate individualized care plans based on clinical data and comorbidities.
- Provide nutrition education and counseling to residents, families, and staff.
- Collaborate with interdisciplinary teams to improve health outcomes.
- Ensure adherence to regulations and company standards.
- Provide coverage across multiple buildings as needed.
- All other duties as assigned.
