Qualifications
- RD credential required and active in good standing.
- State licensure required with the possibility of eligibility where allowed.
- Strong organizational and communication skills expected.
Responsibilities
- Assess the clinical nutrition needs of residents and document care plans in the EMR.
- Develop, implement, and evaluate individualized nutrition care plans based on clinical data.
- Provide nutrition education to residents, families, and staff.
- Collaborate with interdisciplinary teams to optimize residents’ health outcomes.
- Ensure adherence to CMS, state, and federal regulations and company standards.
- Provide coverage across multiple buildings as needed.
