As a Dietitian with L Nutra Health, you will represent our programs in supporting our members with their clinical goals, so they will have a safe and engaging journey to disease remission. Join us on the journey of a new paradigm shift in metabolic health and diabetes care!
Responsibilities
- Manage a panel of clients seeking to prevent or manage chronic metabolic conditions.
- Communicate with clients through video calls, telephone and electronic messaging. Complete counseling in HIPAA compliant platform.
- Meet with L-Nutra Health Chief Medical Officer for weekly Clinical Review sessions.
- Regular outreach to HCPs as needed to obtain patient lab data, blood glucose monitoring instructions, medication adjustment instructions for members, and patient updates.
- Create education materials, handouts, and others as assigned for clients with evidence-based practice and science- based recommendations.
- Provide routine reporting of nutrition counseling sessions including occurred, rescheduled, canceled and no-shows.
- Support coaching Initiatives and ensure compliance on a weekly basis.
- Document in the appropriate time frame.
- Collaborate with other team members and clinicians to oversee program execution.
- Utilize motivational interviewing to support behavior change and enhance program experience, engagement and satisfaction.
- Assess and help develop technical integration of coaching services with the software platform.
- Programs may need to be supported outside normal business hours, which will be determined in advance.
- Develop engaging content related to the Diabetes Program based on L-Nutra’s proprietary technology, including website content and educational materials.
- Provide blog posts (1 per month) and other multi-media content that support the mission and vision of the company.
- Create nutrition education based on needs for our clients.
- Engage in social media community pages.
- Ensure science-based recommendations for patients and customers.
- Provide brief clinical introductions and explanations of the Program to perspective members or HCP’s as needed.
- Continuously look for areas of improvement and communicate feedback and suggestions as needed.
- Represent the brand by embodying L-Nutra's core values and acting in alignment with the mission and vision of the company.
- Partner with L-Nutra Health team to provide feedback on L-Nutra Health Programs on a regular basis.
- Monitor and report member and HCP feedback to enhance program experience and satisfaction.
