Circle Home Inc offers a program of telehealth services focused on helping moderate to high-risk patients (primarily those covered by Medicare) living at home to better manage their care so they can minimize or avoid breakdowns in their health resulting in expensive hospitalizations, emergency department use or other institutional care. The Circle Home Program (CHP) is carried out through a highly specialized, multi-state physician group practice specializing in remote chronic care coordination and monitoring of patients in the home. Care coordination and health status monitoring activities are carried out by a team of nurse practitioners, registered nurses, social workers, and dietitians.
JOB OVERVIEW
The Registered Dietitian is a member of the Circle Home multidisciplinary care coordination team. In collaboration with a multidisciplinary care team, the Registered Dietitian will perform standardized comprehensive needs assessment, identify, and address barriers to care and align patients with available benefits and resources needed to facilitate a plan of care implementation.
JOB RESPONSIBILITIES
Collaborates with members of the care team to facilitate the progression of the patient's care plan goals.
Confers with members of the care team on the patient’s dietary/nutritional situation and needs.
Assesses the patient’s dietary/nutritional situation in relation to the medical and behavioral health needs by providing brief assessments of each patient participating in the CHP.
Interviews patients, family members, and caregivers via telephone to assess dietary/nutritional needs and plan appropriate interventions and goals.
Provides nutrition counseling and education.
Educates patients, family members, and caregivers on how nutrition can affect their condition.
Create personalized eating plans in collaboration with patients, family members, and caregivers.
Works with patients and families to ensure diet and nutritional needs are met to promote health and well-being.
Reviews patient care plans for progression and barriers to meeting goals and collaborates with Circle Home clinical team and referring provider on objectives, interventions, and needed support.
Review remote patient monitoring measurements as appropriate to dietary/nutritional situation.
Perform appropriate reassessments and evaluate progress against care goals and the plan of care and revises plan, as needed.
OTHER RESPONSIBILITIES:
Responsible for attending and actively participating in all staff meetings, in-services, continuing education, and annual compliance requirements.
Comply with all OSHA and safety policies, practices, and procedures. Report all unsafe practices or accidents to supervisor.
Will be compliant with the relevant regulations concerning the privacy and security of patient’s protected health information (PHI) as established by the Health Insurance Portability and Accountability Act of 1996 (HIPAA).
Perform other duties as assigned.
REQUIRED MINIMUM QUALIFICATIONS:
Must be a Registered Dietitian credentialed by the Commission on Dietetic Registration.
Current Licensed Dietitian in Maryland- MUST BE LICENSED IN MARYLAND
Two (2) years of relevant experience; working with the Medicare population highly preferred.
Must have at least 1 year of experience conducting telehealth visits with patients.
Highly proficient in spoken and written English (bilingual is a plus).
KNOWLEDGE, SKILLS, AND ABILITIES:
Demonstrated knowledge and understanding of effective chronic care coordination.
Proficient in documenting in an EMR.
Highly organized and self-directed with strong problem-solving skills.
Exceptional time management abilities
WORK SCHEDULE
The CHP core business hours are Monday- Friday from 8:30 a.m. to 5:00 p.m.; weekly schedules will vary depending on caseload and the needs of the patients. The Registered Dietitian will effectively manage their calendar to maintain contracted hours, and to ensure they are accessible and that the patients’ needs are given the highest priority.

The employee frequently is required to use hands or fingers; handle or feel objects, tools, or controls. The employee is occasionally required to stand; walk; sit; reach with hands and arms. Specific vision abilities required by this position include close vision, distance vision, peripheral vision, and the ability to adjust focus. The noise level in the work environment is usually moderate.
The employee is required to maintain a HIPAA compliant home-based office where there are minimal distractions and patient privacy can be protected.
