Job summary

Healthcare Services Group, Inc. is seeking a Registered Dietitian to oversee clinical nutrition in a long-term care facility. You will assess residents and develop care plans, and provide nutrition education to residents and staff.

This role requires RD certification, state license in good standing, and the ability to work Monday through Friday with flexible schedules across multiple buildings as needed. You will collaborate with interdisciplinary teams, ensure compliance with CMS and state

Qualifications

• Registered by the Commission on Dietetic Registration.
• State license in good standing.
• Knowledge of state survey process (preferred).
• Strong organizational and communication skills.
• Compliance with COVID-19 vaccination policies as required.
• If multi-site, must have a valid Driver's License.
• RD-Eligible and provisional licensure accepted in certain locations.

Responsibilities

• Assess the clinical nutrition needs of residents/patients and document the plan in the EMR.
• Develop, implement, and evaluate individualized care plans based on findings and weight trends.
• Provide nutrition education and counseling to residents, families, and staff.
• Collaborate with interdisciplinary teams to improve health and well-being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs.
• All other duties as assigned.

• Develop, implement, and evaluate individualized care plans based on clinical findings, lab results, weight trends, intake, and comorbidities
• Provide nutrition education and counseling to residents/patients, families, and staff.
• Collaborate with interdisciplinary teams to determine a plan of care that improves residents'/patients’ overall health and well-being.
• Ensure adherence to CMS, State, and Federal regulations and company standards.
• Provide coverage in multiple buildings based on company needs, and as desired.
• All other duties as assigned.

Qualifications

• Registered by the Commission on Dietetic Registration. **
• Hold a Certification/License in good standing within the state of practice.
• Knowledge of the state survey process and compliance with nutrition care regulations (preferred, but not required).
• Strong organizational and communication skills.
• Maintain a positive attitude and effectively communicate with building occupants, administration, and HCSG leadership to perform daily tasks.
• Compliance with COVID-19 vaccination policies as required by state, local, and/or customers.
• If multi-site registered dietitian, must have valid Driver's License
• RD-Eligible and provisional state licensure accepted in certain locations
