Qualifications

  • Registered Dietitian Nutritionist (RDN) credential required.
  • Experience in a hospital or long-term care setting is desirable.
  • Strong knowledge of geriatric nutrition and dietary management techniques.
  • Excellent interpersonal and communication skills with patients and healthcare teams.
  • Familiarity with health coaching principles is a plus.
  • Ability to assess patient needs accurately and develop interventions.

Responsibilities

  • Develop and implement personalized nutrition care plans based on individual health conditions and goals.
  • Provide health coaching to patients, guiding them toward healthier eating habits and lifestyle changes.
  • Monitor and adjust dietary plans as necessary, especially for patients requiring tube feeding or specialized diets.
  • Collaborate with interdisciplinary teams to support the nutritional needs of geriatric patients and those in long-term care facilities.
  • Educate patients and their families about nutrition-related topics, including weight loss strategies and healthy eating practices.
  • Supervise dietary staff as needed, ensuring compliance with established protocols and standards of care.
  • Stay informed about the latest research in nutrition and physiology to provide evidence-based recommendations.
