Job summary

Pivotal Placement Services, Inc. is seeking a Registered Dietitian for a skilled nursing and rehabilitation facility in Chestertown, MD.

You will perform nutrition assessments, create care plans, and work with physicians and the interdisciplinary team to improve resident outcomes. Requires an active Maryland license and a bachelor's degree in Dietetics or related field; experience in long-term care or clinical nutrition is preferred.

Qualifications

• Active Maryland Dietitian/Nutritionist license required.
• Experience in skilled nursing, rehabilitation, long-term care, or clinical nutrition preferred.
• Enteral nutrition experience highly preferred.
• Wound care nutrition and therapeutic diets knowledge strongly desired.
• Strong communication, organizational, and interdisciplinary collaboration skills.

Responsibilities

• Complete comprehensive nutrition assessments for residents upon admission and as clinically indicated.
• Develop, implement, and monitor individualized nutrition care plans.
• Evaluate residents for nutritional risk, including weight loss, wounds, swallowing concerns, and chronic disease management.
• Provide nutrition counseling and education to residents, families, and staff.
• Collaborate with physicians, nursing, speech therapy, and other interdisciplinary team members to ensure coordinated care.
• Monitor residents receiving enteral nutrition and provide recommendations to optimize outcomes.
• Participate in care plan meetings, quality initiatives, regulatory compliance activities, and survey readiness efforts.
