Job summary

Chestertown Nursing and Rehab is seeking a compassionate Registered Dietitian to join our interdisciplinary team. You will assess residents' nutritional needs, develop individualized care plans, and collaborate with physicians, nursing staff, and other healthcare professionals to promote health and quality of life.

Responsibilities include admissions assessments, monitoring nutrition plans, evaluating nutrition risk, and providing counseling to residents, families, and staff.

Qualifications

• Bachelor's degree in Dietetics, Nutrition, or a related field.
• Registered Dietitian (RD/RDN) credential required.
• Current state licensure or certification, if required.
• Experience with enteral nutrition, wound care nutrition, and therapeutic diets preferred.
• Strong organizational, communication, and interpersonal skills.

• Complete comprehensive nutrition assessments for residents upon admission and as needed.
• Develop, implement, and monitor individualized nutrition care plans.
• Evaluate residents for nutritional risk, including weight changes, wounds, swallowing concerns, and chronic disease management.
• Provide nutrition counseling and education to residents, families, and staff.
• Collaborate with physicians, nursing, speech therapy, and other members of the interdisciplinary team to ensure coordinated resident care.
• Monitor residents receiving enteral (tube) feeding and make appropriate recommendations.
• Ensure compliance with all federal, state, and facility regulations related to nutrition services.
• Participate in care plan meetings, quality assurance initiatives, and survey readiness activities.
