Job summary

Job description

Join to apply for the Registered Dietitian role at Spectrum Health Services, Inc.

Responsibilities
  1. Function as a nutrition consultant in a team-based environment, completing referrals from SHS providers.
  2. Integrate nutrition into non-referral patient visits, including routine and unscheduled visits, and warm hand-offs from providers.
  3. Provide all aspects of nutrition care, including medical nutrition therapy (MNT), education, and counseling based on patient needs and standards.
  4. Communicate assessments, reports, and care plans to physicians and multidisciplinary teams, following IDNT standards.
  5. Evaluate assessments, interpret diagnostic and treatment data, and develop, implement, and monitor nutrition care plans.
  6. Review and modify diets to meet patients' dietary needs requiring intervention.
  7. Provide Diabetes Self-Management Support (DSMS) and diabetes prevention education in community settings.
  8. Identify high-risk patients, including those with uncontrolled hypertension or high blood pressure readings without a diagnosis.
  9. Increase awareness of diabetes, pre-diabetes, gestational diabetes, hypertension, and the importance of lifestyle changes.
  10. Document medical records with nutritional status, intake, education, and relevant data.
  11. Assess educational needs, provide counseling, and schedule outpatient visits.
  12. Update knowledge on nutritional therapies for program management, education, and chronic disease care.
  13. Participate in team meetings and develop relationships with external organizations to support patient education.
  14. Maintain licensure and registration through continuing education.
  15. Comply with HIPAA, OSHA, and support Patient-Centered Medical Home (PCMH) concepts.
  16. Provide self-management education for patients with diabetes and perform other duties as assigned.
Requirements
  1. Bachelor’s degree in a related field, with coursework approved by ACEND; Master’s preferred.
  2. At least two years of experience in primary care or community health (preferred).
  3. RD certification via the CDR exam required.
  4. Must obtain state licensure (LDN) within 3 months of hire.
  5. Preferred certifications: CDE, CNSC, CSP.
