Responsibilities
- Function as a nutrition consultant in a team-based environment, completing referrals from SHS providers.
- Integrate nutrition into non-referral patient visits, including routine and unscheduled visits, and warm hand-offs from providers.
- Provide all aspects of nutrition care, including medical nutrition therapy (MNT), education, and counseling based on patient needs and standards.
- Communicate assessments, reports, and care plans to physicians and multidisciplinary teams, following IDNT standards.
- Evaluate assessments, interpret diagnostic and treatment data, and develop, implement, and monitor nutrition care plans.
- Review and modify diets to meet patients' dietary needs requiring intervention.
- Provide Diabetes Self-Management Support (DSMS) and diabetes prevention education in community settings.
- Identify high-risk patients, including those with uncontrolled hypertension or high blood pressure readings without a diagnosis.
- Increase awareness of diabetes, pre-diabetes, gestational diabetes, hypertension, and the importance of lifestyle changes.
- Document medical records with nutritional status, intake, education, and relevant data.
- Assess educational needs, provide counseling, and schedule outpatient visits.
- Update knowledge on nutritional therapies for program management, education, and chronic disease care.
- Participate in team meetings and develop relationships with external organizations to support patient education.
- Maintain licensure and registration through continuing education.
- Comply with HIPAA, OSHA, and support Patient-Centered Medical Home (PCMH) concepts.
- Provide self-management education for patients with diabetes and perform other duties as assigned.
Requirements
- Bachelor’s degree in a related field, with coursework approved by ACEND; Master’s preferred.
- At least two years of experience in primary care or community health (preferred).
- RD certification via the CDR exam required.
- Must obtain state licensure (LDN) within 3 months of hire.
- Preferred certifications: CDE, CNSC, CSP.
