Job summary

Liberty Healthcare and Rehabilitation Services is seeking a Regional Travel Physical Therapist for Liberty Commons of Columbus County in Whiteville, NC. You will evaluate patients, develop written plans of care with measurable goals, supervise PTA/PT aides, and coordinate with the rehab team.

Travel to assigned facilities is required. SNF experience is preferred; a valid driver's license and licensure in the state are a must.

Qualifications

• Evaluates patients and establishes objective, measurable plans of care.
• Supervises PTA/PT aides and coordinates rehab team.
• Maintains accurate daily treatment records and ensures timely documentation.

Responsibilities

• Responds to service requests and obtains physician orders for evaluation.
• Supervises/trains PTA or PT aides.
• Evaluates patients and writes a plan of care with functional goals.
• Plans, administers, and supervises the LPTA after evaluation.
• Maintains daily treatment records.
• May perform in-services at facilities.
• Maintains a productive workload (≥75%).
• Establishes restorative/maintenance programs.
• Adjusts plans of care as needed.
• Maintains weekly progress notes and adheres to documentation guidelines.
• Prepares and maintains work setting, equipment, and supplies.
• Communicates with referring physician and rehab team.
• Follows Liberty Long Term Care procedures.
• Submits employee paperwork to corporate timely.
• Willing to travel to assigned locations, including overnight stays.
