Job summary

Presbyterian Homes & Services (PHS), based in St. Paul, Minnesota, is seeking an RAI Director to coordinate resident assessments across care centers in a regional role. The position is hybrid with on-site visits to communities and reports to the Director of Care Center Services, supervising MDS nurses.

The ideal candidate will have RN licensure, long-term care RAI/MDS experience and strong leadership to drive regulatory compliance and clinical best practices.

Qualifications

• Registered nurse with current licensure in state(s) of practice.
• Experience in long-term care and working knowledge of the RAI process.
• Experience with MDS completion, reimbursement, and case management.
• Ability to work flexible hours to support business requirements.
• Competency with standard office software and MDS/RAI software.

Responsibilities

• Hire, train and manage MDS Nurses in the assigned region.
• Direct and ensure development and completion of the resident assessment process.
• Maintain accurate schedules and assignments for MDS assessments.
• Audit and review PDPM/MDS processes and related appeals.
• Lead monthly QI/QM review and provide recommendations.
• Coordinate with IDT to optimize care plans for the region.
• Communicate Medicare/Medicare Advantage changes and PPS-related notices.
• Lead work groups and stay current with QRP/Quality Measures.
