Job summary

VNS Health in New York seeks a Nurse Practitioner to provide advanced nursing care within home and community settings, under the direction of the Clinical Director for the Nurse Practitioner Program. You will lead inter-professional teams, manage care plans, and ensure compliance with state and federal standards.

The role includes post-discharge visits, on-boarding for palliative care programs, and on-call coverage to ensure 24/7 access to care.

Qualifications

• Master's Degree of Science in Nursing is required.
• ANCC or AANP certification as an adult, family or geriatric NP is required.
• New York NP license/registration and credentialing are required

Responsibilities

• Lead advanced nursing practice for targeted patient populations and evaluate responses to therapy.
• Serve as clinical authority on care teams and lead rounds and interdisciplinary meetings.
• Provide comprehensive advanced nursing care including post-discharge visits and care-management onboarding.
• Collaborate with patients, families and primary care physicians to plan care.
• Manage care across settings and ensure quality measures compliance and proper documentation.
