Qualifications

• Experience in a healthcare delivery setting with elderly patients; nursing home preferred.
• Excellent interpersonal and communication skills; ability to work independently or with a team.
• Ability to collaborate with nursing staff, doctors, and administration across facilities.
• Track record of delivering outstanding customer service with CMS regulation compliance.
• Willingness to travel to facilities within the assigned region and maintain reliable transportation.
• Proficiency with Microsoft Office and reporting tools; strong organizational skills.

Responsibilities

• Coordinate with Insurance Producer to ensure seamless enrollment-to-service transition.
• Follow up with members to answer questions and provide plan-related assistance.
• Serve as front-line contact for members, families, and care teams to meet plan needs.
• Develop and maintain relationships with network providers within the region.
• Meet with members or families at least monthly to review status and issues.
• Maintain reporting on member status, facility calendars, and issues tracking.
• Ensure interactions comply with CMS regulations and plan requirements.
• Travel to nursing facilities within the assigned region as needed.
