Job summary

Abode Care Partners seeks a Regional Medical Director to lead clinical strategy across post-acute settings, including SNFs and home-based care. The role balances active clinical practice (25–50%) with region-wide leadership, quality improvement, and stakeholder collaboration.

You will report to the Chief Medical Officer and drive standardized care pathways, provider performance, and utilization management in a distributed, multi-site environment.

Qualifications

• MD/DO degree from an accredited medical school.
• Completion of an accredited residency in Internal Medicine, Family Medicine, Geriatrics, or related field.
• Board certification in an adult primary care specialty is strongly preferred.
• Five+ years of post-residency clinical experience preferred, with geriatrics exposure.
• Three+ years of progressive physician leadership experience preferred.

Responsibilities

• Provide regional clinical leadership and accountability.
• Oversee quality of care and adherence to the care model.
• Manage high-risk populations and prevent avoidable hospitalizations.
• Educate and develop clinicians, including physicians and APPs.
• Ensure documentation, coding, and clinical record quality.
• Lead post-acute, SNF, and assisted living leadership efforts.
• Collaborate with operations on policy, strategy, and growth.
• Engage in value-based care, population health initiatives.
