Regional Medical Director / Geriatric Primary Care Provider

Full-Time | Monday–Friday, 40 hours/week | Shared Call Schedule | Start Date: September 1

Locations: Kokomo, IN & Jeffersonville, IN

We are seeking an experienced and mission-driven Regional Medical Director/Geriatric PCP to join our growing, patient-centered team. In this dual clinical and leadership role, you will oversee the delivery of high-quality medical care for older adult populations, while also providing direct primary care services. This position offers a unique opportunity to make a meaningful impact on care delivery in a collaborative, team-oriented environment.

Key Responsibilities:

• Serve as the regional clinical leader, setting the standard for quality and innovation in care delivery
• Function as a Primary Care Provider (PCP) for a panel of geriatric patients, delivering comprehensive, compassionate care
• Oversee medical operations across multiple clinic sites; some travel required (1–2 times per month) between Kokomo and Jeffersonville
• Supervise and support an interdisciplinary team of Nurse Practitioners (NPs), Registered Nurses (RNs), and other providers
• Collaborate with the leadership team on performance improvement strategies, patient safety initiatives, and program development
• Develop, review, and implement medical care policies and best practices
• Chair the Quality Subcommittee, working to enhance patient satisfaction and clinical outcomes
• Foster strong relationships with external network providers and community partners
• Act as a mentor and resource for clinical staff, promoting a culture of continuous learning and improvement
• Team-based care model with clinics staffed by experienced NPs and RNs
• EHR System: Carehub
• Collaborative administrative and operational support
• Regular clinical leadership meetings and professional development opportunities
• Emphasis on holistic, preventive care and value-based outcomes

Qualifications:

• MD or DO with Board Certification in Internal Medicine or Family Practice
• Minimum of 5 years of clinical experience in geriatrics, senior care, or a related field
• Proven experience in a clinical leadership or medical director role
• Strong track record of working effectively within interdisciplinary teams
• Excellent communication, supervisory, and organizational skills
• Ability to travel occasionally within the assigned region (2-3 times/month)
