Job summary

Legacy Healthcare in Illinois is seeking a Regional MDS Nurse Consultant to support facilities within the region, working in a hybrid schedule. You will develop, monitor, audit, and modify each resident’s care plan, perform resident assessments, and assist in the discharge process to ensure high standards of care.

Qualifications

• Bachelor’s degree in nursing, RN licensed.
• Specific training in gerontological nursing preferred.

Responsibilities

• Collaborates with Director of Clinical Services, Regional Director of Operations, Administrators, and Directors of Nursing within the region.
• Identifies and monitors centers with greatest need for quality and regulatory intervention.
• Leads center and regional operations personnel and staff to improve care delivery, survey results, and reduce penalties.
• Works with local/state/federal agencies and external resources to facilitate clinical competency.
• Fosters nursing leadership support network and spreads performance improvement initiatives.
• Pilots and replicates promising practices.
