Job summary

Opco-Skilled-Management- is seeking an experienced MDS Consultant to lead the development, implementation, coordination and evaluation of MDS services across multiple facilities. The role emphasizes quality care and fiscal responsibility through comprehensive training and program reviews in compliance with federal and state laws.

The position reports to a senior clinician and requires extensive multi-facility MDS experience, strong regulatory knowledge, and the ability to lead a regional

Qualifications

• Currently licensed as RN in the state practiced.
• Associate or bachelor’s degree from an accredited nursing school required.
• Minimum of five years in long-term or acute health care.
• Minimum of five years working as an MDS Nurse in long-term or acute health care.
• At least three years of multi-facility, regional MDS experience.

Responsibilities

• Consults with and provides technical assistance to the MDS Coordinators through visits and the interpretation of policies and regulations.
• Trains new MDS Coordinators in conducting resident assessments and documentation.
• Trains on the RAI manual and deadlines for resident assessments and MDS completion.
• Observes MDS practices for compliance with standards and regulations.
• Inspects facilities for compliance with nursing standards and regulations.
• May assume interim MDS Coordinator as needed.
• Train facility MDS on company best practices with compliance team coordination.
• Lead facility staff in developing a business plan for clinical operations.
• Develop and monitor quality metrics for care and outcomes.
• Advise Clinical Support Team and DON to maximize resident well-being.
• Develop standardized processes to reduce variability in care and improve safety.
• Monitor QM measures data collection and reporting.
• Assess eligibility, referrals, case management and disease management systems.
• Review clinic information system capabilities for standardized reporting.
• Ensure RAI guidelines in the region and timely completion of assessments.
• Utilize survey information to address priorities defined with partners.
• Ensure regulatory compliance and guide facilities per company policies.
• Maintain facility standards and safety; address hazards; follow infection control.
• Prepare staff for inspections and respond to survey findings.
• Participate in Plan of Correction responses and QA follow-ups.
• Provide 24-hour on-call service to nursing centers.
• Ensure orientation and ongoing training for MDS personnel.
• Other duties may be assigned.
