The primary purpose of this role is to maintain and process MDS data, resident medical records, and health information systems in accordance with state and federal requirements, as well as the policies and goals of all facilities. The coordinator is a key participant in the interdisciplinary team, assisting in ensuring quality of care, continuity of services, and individualized patient-centered plans and goals.

Responsibilities
