Qualifications

Regional MDS Coordinator Summary:

As the MDS Coordinator, you’ll serve as the Resident Assessment Instrument (RAI)/Minimum Data Set (MDS) Coordinator for our centers. Your responsibilities will include reviewing and processing MDS assessments in accordance with CMS and LTCI regulations and guidelines.

Responsibilities:

Your impactful contributions will include:

  1. Coordinating the development and revision of resident Interdisciplinary Care Plans based on RAI/MDS information.
  2. Collaborating with nursing caregivers and other disciplines to ensure accurate care plan goals.
  3. Effective communication with various disciplines to facilitate timely completion and transmission of RAI/MDS.
  4. Preparing accurate reports on Quality Measure/Quality Indicators (QM/QI) from RAI/MDS data.
  5. Active involvement in Quality Improvement initiatives.
  6. Implementing educational plans to address changing program or service needs.

Requirements:

  • RN License in the State of Georgia.
  • Extensive experience conducting MDS Assessments.
  • Available to travel regularly throughout the state of Georgia and Florida.
