Job summary
Care Hospice seeks an experienced Regional Hospice RN to support multiple locations across Virginia. You will provide direct patient care, case management, admissions, and coverage based on program priorities, traveling between Charlottesville, Fisherville, Fredericksburg, Harrisonburg, Newport News, Virginia Beach, Richmond, and Winchester.
Must live in Virginia and be a licensed RN. The role offers a salaried, direct-hire position with comprehensive benefits and opportunities to impact care
Qualifications
- Current unrestricted RN license in good standing.
- Willingness to travel 100% within Virginia.
- Minimum 2 years of RN experience.
- Minimum 1 year of hospice/palliative care experience.
- CPR certification if state mandated.
- Experience with home or community-based settings.
- Hospice case management/admissions experience preferred.
Responsibilities
- Provide hospice care across multiple locations and serve as a clinical resource.
- Support patient care delivery through case management, admissions, and other nursing duties.
- Step into high-impact assignments to support staffing, growth, and operations.
- Build relationships with patients, families, physicians, and teams in new environments.
- Deliver assessments, symptom management, care planning, and patient education.
- Collaborate to promote quality outcomes, regulatory compliance, and excellent patient experiences.
- Share expertise and support team success and best practices.
- Maintain accurate documentation in EMR systems.
- Provide feedback to strengthen clinical operations.
- Travel throughout the assigned region.
