Job summary

Transitions Hospice in Elk Grove Village, IL seeks an experienced Director of Hospice Operations to lead regional operations, staffing, and clinical outcomes. The role reports to the Administrator and drives growth, quality, and financial performance across multiple sites.

You will oversee staffing, compliance, and performance metrics, partner with finance, and build partnerships with healthcare providers to expand census and referrals.

Qualifications

• Bachelor's degree in business administration, health care administration, or related field required; Master's degree preferred.
• 5+ years of progressive hospice or home health leadership experience required.
• Demonstrated track record of driving census and organizational growth.
• Intimate knowledge of Medicare Hospice Certification requirements, CHAP standards, and state and federal hospice regulations.
• Proven ability to supervise and direct clinical and administrative personnel across multiple sites.
• Strong understanding of performance improvement, quality assessment, and utilization management.

Responsibilities

• Owns region growth: census, referrals, and market share.
• Identifies partnerships and referral relationships (physicians, hospitals, SNFs, assisted living, community organisations).
• Partners with the Administrator and Finance on regional budgeting, forecasting, and business planning.
• Owns regional performance metrics (census, admissions, length of stay, discharges, revenue) and reports progress and recommendations to the Administrator.
• Represents the organisation in the community and with external stakeholders, strengthening reputation and presence.
• Leads quality, clinical, and compliance outcomes across the region under the Administrator’s supervision.
• Ensures regional compliance with federal, state, CHAP, and CMS regulations and standards.
• Leads ongoing QAPI activities across the region as part of a unified program under the Administrator’s oversight.
• Establishes quality benchmarks, tools, and best practices for consistent standards across all sites.
• Ensures interdisciplinary care is delivered consistently and staff stay current with regulations.
• Escalates compliance concerns identified through QAPI, complaints, audits, or otherwise via the hospice’s compliance program.
• Directs day-to-day regional operations, including staffing, scheduling, supplies, equipment, and technology.
