Carease Health is a nationwide preventative healthcare company focused on reducing hospitalizations through remote monitoring, chronic care management, and collaborative care. We are seeking a Healthcare Regional Manager to oversee preventive care programs across skilled nursing facilities and ensure high-quality service delivery, strong leadership, and regulatory compliance.

The role requires travel to multiple SNFs, coordination with DONs and Administrators, and effective on-site team support.

Qualifications

• Experience in Skilled Nursing Facilities (SNFs) is strongly preferred.
• Prior multi-site management or regional leadership experience is a plus.
• Strong interpersonal and problem-solving skills are essential.
• Ability to educate staff and give workshops.
• Willingness to travel regularly.
• Ability to work independently while aligning with corporate objectives.
• Degree in healthcare-related field preferred.

Responsibilities

• Oversee Service Delivery: Ensure successful implementation and day-to-day operation of Carease's preventative care programs within assigned SNFs.
• Build Strong Relationships: Serve as the primary contact for facility leadership (DONs and Administrators).
• Ensure Regulatory Compliance: Oversee adherence to CMS guidelines and internal protocols.
• Support On-Site Teams: Integrate Carease providers, care companions, and CNAs into facility workflows.
• Conduct Facility Visits: Travel to facilities to monitor performance and address issues.
• Lead Monthly Strategy Meetings: Review metrics and align on goals.
• Resolve Technical Issues: Escalate tech concerns to prevent service disruption.
