Job summary

Carease Health is a nationwide preventive healthcare company focused on reducing hospitalizations. We seek a Healthcare Regional Manager to ensure the smooth delivery of our preventive care programs across a group of Skilled Nursing Facilities, supporting on-site staff and maintaining strong partnerships at the facility level.

You will travel regularly to facilities, oversee program implementation, and collaborate with DONs, Administrators, and care teams to ensure high-quality resident outcomes

Qualifications

• Experience with SNFs or long-term care settings is preferred.
• Strong relationship-building with facility leadership.
• Ability to navigate clinical and operational conversations.

Responsibilities

• Oversee service delivery and day-to-day operations in assigned SNFs.
• Build and maintain relationships with DONs and Administrators.
• Ensure adherence to CMS guidelines and care protocols (RPM, CCM, CoCM).
• Support on-site teams and integrate Carease providers into facility workflow.
• Conduct regular facility visits to monitor performance and address issues.
• Lead monthly strategy meetings to review metrics and goals.
• Resolve technical issues quickly to prevent service disruption.
