Job summary

April Parker Foundation is seeking Regional Care Managers to deliver In-Person Enhanced Care Management (ECM) and Community Supports (CS) to Medi-Cal members with complex needs. This field-based role requires in-person visits and collaboration with health plans and community partners.

You will address physical and behavioral health, as well as social determinants like housing, food access, and transportation, with monthly in-person engagements and flexible scheduling.

Qualifications

  • Minimum 2 years of experience in case management, care coordination, or social services.
  • Knowledge of Medi-Cal / CalAIM programs, community resources, and health determinants.
  • Strong documentation and organizational skills.
  • Excellent communication and relationship-building skills.
  • Access to reliable transportation.

Responsibilities

Location: Los Angeles / Orange County - Field-Based Client Services

Employment Type: Hourly, Full-Time, Non-Exempt

Compensation: Base Hourly wage + Performance Incentives

About the role

The April Parker Foundation is seeking compassionate, detail‑oriented Regional Care Managers to deliver In-Person Enhanced Care Management (ECM) and Community Supports (CS) services to Medi‑Cal members with complex medical, behavioral, and social needs.

This field‑based position is ideal for professionals who value flexibility, autonomy, and meaningful impact. Regional Care Managers are assigned a territory and engage clients in person to address their physical health, behavioral health, and social determinants such as housing, food access, and transportation.

In‑person engagement is required, with a minimum of one in‑person visit per member per month within your assigned region.

  • Ensure compliance with program requirements
  • Support members in achieving long‑term stability and self‑sufficiency
How Compensation Works

We offer a transparent, performance‑driven compensation model that rewards active engagement and in‑person service delivery.

Base Pay: $25.00 per hour for all hours worked. (Approx. $4,000 per month based on 40 hrs/week)

In-Person Performance Incentive Pay: $40.00 per member per month for each member who receives a qualified in‑person ECM or CS service. Care Managers who consistently serve a caseload of 40–60 active members can earn up to $6,400 per month.

Reimbursements & Stipends: Mileage at IRS rate and approved work‑related expenses.

Schedule

Monday–Friday, with occasional weekend and evening availability as required. The majority of hours are field‑based; this is not a home‑office role.

Qualifications

Required:

  • Minimum 2 years of experience in case management, care coordination, or social services
  • Knowledge of Medi‑Cal / CalAIM programs, community resources, and social determinants of health
  • Strong documentation and organizational skills
  • Excellent communication and relationship‑building skills
  • Access to reliable transportation

Preferred:

  • Bachelor’s degree in a related field
  • Fluent Spanish speaker
  • Community Health Worker (CHW) certification
  • California licensure (LCSW, LMFT, LPCC, RN, or similar)
