Job summary

Community Care Alliance in Woonsocket, RI is seeking a Regional Access Point Navigator (Part Time) to work with homeless or at‑risk clients using Housing Problem Solving Techniques to divert from shelter placement. You will coordinate with shelters for openings and update regional access point systems.

The role requires completing intake paperwork, updating living situations in HMIS, maintaining program data, and serving as a referral source for social services.

Qualifications

  • High School Diploma required.
  • Experience with case management and HMIS preferred.
  • Ability to coordinate with multiple service providers.

Responsibilities

  • Schedule and attend regular case management meetings.
  • Complete intake paperwork and ensure documentation is filed.
  • Update living situations and housing assessments in HMIS.
  • Maintain data in HMIS and Smartsheet.
  • Make referrals to applicable social services.
  • Collaborate with providers to address housing barriers.
  • Attend team meetings and participate in PQI efforts.

Regional Access Point Navigator (Part Time)
POSITION SUMMARY:

Interact with homeless or at risk of homeless clients using Housing Problem Solving Techniques in order divert from shelter placement. Coordinate with shelters for any potential openings. Update Regional Access Point Smartsheet and claim any open beds.

Update Current Living Situation and Housing assessment in HMIS (Homeless Information System), confirm and maintain program data. Perform Home Stabilization Intake paperwork and Medicaid Authorization stabilize their living situation and/or assist them to transition into permanent supported housing opportunities. Act as referral source for social services.

Responsibilities
  • Schedule and attend regular case management meetings with program participants.
  • Complete intake paperwork, as needed, ensuring all required documentation is received and filed.
  • Complete current living situations, housing assessments in HMIS.
  • Maintain all required data for services provided in HMIS, and Smartsheet.
  • Make referrals to other appropriate services to address any support needs or barriers to housing.
  • Collaborate with providers offering services to the client.
  • Maintain time allocation record.
  • Work collaboratively with team to achieve goals.
  • Participate in PQI effort of the team.
  • Attend team meetings.
  • Attend weekly supervision meetings and seek consultation when appropriate.
  • Any other duties as assigned by supervisory staff.
EDUCATION AND TRAINING REQUIRED:

High School Diploma.
