Job summary

LifeLong Medical Care seeks a Referrals Coordinator for the Brookside San Pablo Health Center. The role supports a patient-centered care team by processing specialty referrals, following up, and ensuring accurate data entry in Epic and i2i.

Responsibilities include verifying insurance, obtaining authorizations, and coordinating with providers and Health Information to place reports in the chart. 40 hours weekly; benefit eligible; represented by SEIU-UHW under a CBA.

Qualifications

  • High school diploma or GED.
  • One year of administrative experience in a medical or human services setting.
  • Experience with electronic health records (EHR).
  • Proficient in Microsoft Office Word, Excel, Outlook and Internet; ability to type 40 wpm.

Responsibilities

  • Receive referral requests from providers and process them; communicate with providers as needed.
  • Request urgent appointments as needed.
  • Verify current insurance information.
  • Contact HMOs, PPOs, Medi-Cal/Medicare for authorizations to ensure reimbursement.
  • Follow up on referrals and maintain accurate tracking in Epic and i2i.
  • Coordinate with Health Information Technician to ensure medical reports are added to the chart.

LifeLong Medical Care is looking for a Referrals Coordinator at our Brookside San Pablo Health Center. The Referrals Coordinator is part of a patient-centered care team. Responsibilities include providing specialty referrals, follow-up, data entry and tracking.This is a full time, 40 hrs/wk, benefit eligible position.This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.LifeLong Medical Care is a multi-site, Federally Qualified Health Center (FQHC) with a rich history of providing innovative healthcare and social services to a wonderfully diverse patient community.
