Responsibilities

• Review open referrals and search for related reports in patients' medical records.
• Communicate with specialists/imaging facilities to ensure patient appointment compliance, document in notes.
• Request consultations/imaging reports and follow up until reports are received.
• Inform Referral Coordinator of issues related to communications with specialists.
• Communicate with patients who did not attend appointments and direct them to reschedule.
• Follow procedures for referrals no longer wanted or when unable to reach the patient.
• Assist as needed in duties of Referral Specialist.
• Perform additional duties as directed by the Referral Coordinator.

Description

The Referral Tracker is responsible for maintaining ongoing tracking and appropriate documentation on referral status. Connects with offices/ facilities and requests the notes/ reports for continuity of care and reaches out to patients to reschedule appointment as needed.

Requirements

• Requires a high school diploma or equivalent.
• A minimum of 2 years of experience in a medical office preferred. A minimum of one (1) year experience as a Referral Specialist with ESRH required.
• Individual must be professional, self-motivated and have strong attention to detail, communication, teamwork, and computer skills.
• Must demonstrate excellent internal and external customer service skills.
• Provide coverage at other centers, or for other providers, on short notice when needed.
• Certification as an Interpreter is preferred and ability to provide bilingual patient care is a plus.

Summary

• Review open referrals and search for related reports that may already be in the patients’ medical records. Address referral if report is found.
• Communicate with specialists/ imaging facilities to determine patient compliance with attending appointment. Document compliance in notes tab of the referral.
• Request consultations/ imaging reports for appointments attended. Continue to communicate with specialist/ imaging facilities as needed until all reports are received. Address referral once the report is received.
• Inform Referral Coordinator of issues related to communications with specialist/imaging facilities.
• Communicate with patients who did not attend scheduled appointment to determine if referral is still warranted and direct the patient to reschedule if still needed.
• Follow the Referral Initiation, Scheduling and Tracking Procedure (C PRO 80) for addressing referrals that are no longer wanted or if unbale to reach patient.
• Assume the duties of a Referral Specialist when needed.
• Other duties and tasks as directed by the Referral Coordinator.

ADDITIONAL DUTIES ASSIGNED TO DESIGNATED BILINGUAL STAFF

• If qualified, perform interpretation/translation services for ESRHS centers.
