A Referral Specialist is needed at Advocate Health in Park Ridge. You will support providers and patients within the ambulatory care team, handling centralized referrals and discharge planning to align with benefit coverage.

The schedule is 8:00am-4:30pm or 8:30am-5:00pm, Monday through Friday. The role requires AA degree or equivalent and 3-4 years in a managed care setting; prior referral experience strongly preferred.

As a key member of the ambulatory care team, you will:

• Management and navigation of the centralized referral process.
• Responsible for the coordination and monitoring of discharge planning to provide cost effective care and quality outcomes to members in accordance with their benefit coverage guidelines.
• Participate in education and utilization management activities
