Palm Medical Centers in Winter Haven, FL is hiring a full-time Referral Coordinator to manage referrals, register patients accurately, and coordinate with insurers for prior approvals, ensuring timely access to care.

The ideal candidate will have strong communication and organizational skills, be bilingual in English and Spanish, and be proficient with EMR systems and MS Office. This is an on-site role with a comprehensive benefits package.

Qualifications

• High school diploma or equivalent required.
• Minimum 1 year experience with referrals or medical office work.
• Experience with EMR systems is preferred.

Responsibilities

• Track referrals and document them to promote team awareness and patient safety.
• Register patients accurately, including demographics and insurance.
• Enter and update referral data in the electronic system.
• Gather information on patient's clinical background and referral needs.
• Coordinate with review organizations and insurers for prior approvals.
• Provide medical information to financial services to aid reimbursement.
• Explain referral details to patients and address questions.
• Assist patients with barriers to care, including interpreters or transportation.
• Build relationships with community resources and service providers.
• Ensure referrals are addressed promptly and chart is up to date.

Job description

Duties & Responsibilities:

• Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
• Ensure complete and accurate registration, including patient demographics and current insurance information.
• Must enter and update referral data in electronic system as appropriate.
• Assemble information concerning patient's clinical background and referral needs.
• Contact review organizations and insurance companies to ensure prior approval requirements are met.
• Present necessary medical information such as history, diagnosis and prognosis.
• Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
• Review details and expectations about the referral with patients.
• Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).
• Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
• Ensure that referrals are addressed in a timely manner.
• Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.'

Education and Experience:

• High school diploma or equivalent required. Minimum requirement of one year work experience with referrals, or related medical office experience.
• Experience with EMR systems is preferred.

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift up to 10 pounds at times.
