Job summary

Palm Medical Centers in Winter Haven, FL is seeking a full-time Referral Coordinator to manage referrals, registrations, and prior authorization processes. You will enter and maintain referral data in the EMR, communicate with patients, insurers, and review organizations, and help patients navigate financial or transportation barriers.

The role requires strong communication, organization, and bilingual English/Spanish abilities, with standard healthcare office experience preferred.

Qualifications

• High school diploma or equivalent required.
• At least 1 year experience with referrals or in a medical office.
• EMR systems experience preferred.

Responsibilities

• Track and document referrals to promote team awareness and patient safety.
• Register patients accurately with demographics and current insurance information.
• Enter and update referral data in the electronic system.
• Assemble information about patient clinical background and referral needs.
• Coordinate with review organizations and insurers for prior approvals.
• Present medical information such as history, diagnosis and prognosis.
• Provide medical information to financial services to maximize reimbursement.
• Review referral details with patients and explain expectations.
• Assist patients with problem solving health care, financial or social barriers.
• Identify community resources and maintain provider relationships.
• Ensure referrals are addressed in a timely manner.
• Keep PCP chart updated with specialist consultations and hospital visits.

Job description

Duties & Responsibilities:

• Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.
• Ensure complete and accurate registration, including patient demographics and current insurance information.
• Must enter and update referral data in electronic system as appropriate.
• Assemble information concerning patient's clinical background and referral needs.
• Contact review organizations and insurance companies to ensure prior approval requirements are met.
• Present necessary medical information such as history, diagnosis and prognosis.
• Provide specific medical information to financial services to maximize reimbursement to the hospital and physicians.
• Review details and expectations about the referral with patients.
• Assist patients in problem solving potential issues related to the health care system, financial or social barriers (e.g., request interpreters as appropriate, transportation services or prescription assistance).
• Identify and utilize cultural and community resources. Establish and maintain relationships with identified service providers.
• Ensure that referrals are addressed in a timely manner.
• Ensure that patient's primary care chart is up to date with information on specialist consults, hospitalizations, ER visits and community organization related to their health.'

Education and Experience:

• High school diploma or equivalent required. Minimum requirement of one year work experience with referrals, or related medical office experience.
• Experience with EMR systems is preferred.

Physical Requirements:

Prolonged periods of sitting at a desk and working on a computer.
Must be able to lift up to 10 pounds at times.
