Job summary

The role focuses on referral screening, patient guidance, and tracking referrals to ensure timely access to care. You will document outcomes, generate periodic reports, and coordinate with clinics to resolve access issues, while meeting requirements such as a background check, probation, immunization screening, and a 5% TDY travel

Qualifications

• Background check may be required.
• 1-year probationary period unless previously met.
• Immunization screening required due to occupational risk.
• Mandatory annual influenza vaccination.
• Mission Essential designation; report to work during emergencies.
• 5% TDY travel requirement.

Responsibilities

• Screen and appoint internal referrals in a timely and accurate manner.
• Guide patients on how to receive care requested in referrals and document results.
• Track completion of internal and external referrals.
• Prepare routine reports to determine referral verification or closure rates.
• Identify clinics with access issues and communicate with specialty staff to address them.
• Counsel beneficiaries on accessing care via MTF, network, or TRICARE/benefits.
