Job summary

EAC Network is seeking a dedicated full-time Referral and Assessment Coordinator to support the Jail Population Review Program. The role focuses on managing referrals, coordinating assessments, and collecting collateral documents to ensure timely and accurate reporting.

The position emphasizes working with clinical staff, attorneys, and correctional contacts while maintaining strict confidentiality. Location is Brooklyn, NY, with standard daytime hours.

Qualifications

• High school diploma required.
• Associate's degree preferred.
• Minimum of 2 years of administrative, case coordination, legal, behavioral health, criminal justice, or human services experience.

Responsibilities

• Receive, log, and track referrals for individuals referred through the Jail Population Review process.
• Maintain referral and assessment tracking systems with dates, status, and outcomes.
• Coordinate with clinical staff to identify required collateral information (medical, correctional health, legal records, etc.).
• Request, collect, organize, and upload collateral documents following procedures and confidentiality rules.
• Schedule assessment interviews by video conference when possible, and guide sources and staff on access and logistics.
• Coordinate in-person interview logistics when video is not available, including location details and contacts.
• Communicate with attorneys, courts, correctional contacts, and staff to support timely completion of assessments.
• Monitor deadlines and alert leadership to pending items or delays.
• Maintain accurate documentation of case activity, outreach attempts, and outcomes.
• Assist with administrative reports, summaries, and data submissions as required.
• Ensure all records comply with HIPAA and agency policies.
