Job summary

EAC Network in Brooklyn, NY seeks a full-time Case Manager for the Brooklyn Treatment Alternatives for Safer Communities program. The role focuses on court liaison, assessment, referral and monitoring of clients in the criminal justice system into appropriate treatment modalities.

You will manage a caseload and provide cognitive behavioral interventions and HIV/Hepatitis resources. Required is an Associate Degree and at least 2 years of experience in intervention programs with justice-involved

Qualifications

• Associates degree in a helping profession and at least 2 years of experience in intervention programs with criminal justice involved participants.
• Bachelor's degree preferred.
• Strong communication skills and ability to work with treatment and justice systems.

Responsibilities

• Act as a liaison to the courts and arrange placement into treatment programs.
• Complete follow-up paperwork and maintain client records for audits.
• Prepare monthly and quarterly progress reports for funders.
• Prepare and present status reports to the criminal justice system regarding client progress.
• Arrange supportive services for clients.
• Escort clients to appointments and treatment facilities when needed.
