Responsibilities

  • Receive, log, and track referrals for individuals referred through the Jail Population Review process.
  • Maintain referral and assessment tracking systems with dates, status, and outcomes.
  • Coordinate with clinical staff to identify required collateral information (medical, correctional health, legal records, etc.).
  • Request, collect, organize, and upload collateral documents following procedures and confidentiality rules.
  • Schedule assessment interviews by video conference when possible, and guide sources and staff on access and logistics.
  • Coordinate in-person interview logistics when video is not available, including location details and contacts.
  • Communicate with attorneys, courts, correctional contacts, and staff to support timely completion of assessments.
  • Monitor deadlines and alert leadership to pending items or delays.
  • Maintain accurate documentation of case activity, outreach attempts, and outcomes.
  • Assist with administrative reports, summaries, and data submissions as required.
  • Ensure all records comply with HIPAA and agency policies.

Salary: $26-30/hour

Schedule: 9AM-5PM, Monday - Friday

Location: 175 Remsen Street, 5th Floor in Brooklyn, NY

What drives the core purpose of this job:

The Referral and Assessment Coordinator supports the Jail Population Review initiative by managing referral tracking, assessment scheduling, collateral document collection, and outcome documentation. The position ensures that referrals move efficiently through the assessment process and that clinical staff have the records, scheduling information, and administrative support needed to complete timely assessments and reports.

What are the key responsibilities:
  • Receive, log, and track referrals for individuals referred through the Jail Population Review process.
  • Maintain referral and assessment tracking systems, including referral date, assignment, due dates, assessment status, report completion, and outcomes.
  • Coordinate with clinical staff to identify required collateral information, including medical records, correctional health information, legal documents, criminal history, treatment records, and other relevant documentation.
  • Request, collect, organize, and upload collateral documents in accordance with program procedures and confidentiality requirements.
  • Schedule assessment interviews by video conference when appropriate and provide clear instructions to referral sources, attorneys, facility contacts, and staff regarding access and logistics.
  • Coordinate in-person interview logistics when video conference is not appropriate or available, including instructions, location details, required contacts, and documentation needed.
  • Communicate with attorneys, courts, correctional contacts, CJI staff, treatment providers, and internal staff to support timely assessment completion.
  • Monitor assessment deadlines and alert the Program Director and clinical staff regarding pending due dates, missing documents, scheduling barriers, or delayed cases.
  • Maintain accurate documentation of case activity, outreach attempts, scheduled interviews, received records, outstanding items, and final outcomes.
  • Support preparation of administrative reports, referral summaries, outcome tracking, and data submissions as required.
  • Ensure all records and communications comply with HIPAA, confidentiality requirements, agency policies, and program protocols.
  • Assist with tracking consultant invoices, ensuring they are complete and accurate, and submitting them to Finance in a timely manner for processing.
  • Perform other duties as assigned.

  • Ability to manage multiple referrals, deadlines, and follow-up tasks at the same time.
  • Strong written and verbal communication skills.
  • Ability to communicate professionally with attorneys, courts, correctional contacts, funders, providers, and internal staff.
  • High attention to detail in data entry, documentation, and file organization.
  • Ability to handle confidential information appropriately.
  • Familiarity with Microsoft Office, Outlook, Excel, Teams, SharePoint, or similar systems.
  • Ability to identify missing information and elevate barriers timely.
Minimum Qualifications:
  • High school diploma or equivalent required.
  • Associate's degree preferred.
  • Minimum of 2 years of administrative, case coordination, legal, behavioral health, criminal justice, or human services experience.
