Job summary

The Kintock Group, Inc. is seeking a case management professional to deliver prerelease and post-release services. You will travel to state prisons, jails, and parole offices to recruit eligible participants and provide intake, assessments and ongoing case management in the community as needed.

You will develop individualized plans, connect clients with social services, and document all activities per state regulations, while attending required trainings and collaborating with the Director.

Qualifications

  • Four-year degree in a social, human services, criminal justice or behavioral science program from an accredited college or university.
  • At a minimum, one year of experience in human services, corrections, or community-based services.
  • Work experience may be substituted for academic studies exchanging one year of work experience in a related field for one year of academic education.
  • Total work experience needed in lieu of the combination of education and work experience is five years.
  • Valid NJ Driver’s license.

Responsibilities

  • Provide prerelease (inside state prisons) and post-release (in the community) case management services.
  • Travel to the State Prisons, County Jails, Probation & Parole offices and other criminal justice service providers to promote and recruit inmates who are eligible for the program.
  • Provide intake, assessment and case management services to clients, including ongoing assessment for program eligibility, monitoring all aspects of client services, ensuring needs of clients are met; individual counseling and client educational classes may be included at discretion of the Director.
  • Develop a comprehensive plan to meet the needs of clients releasing to the identified counties of need.
  • Provide the client with information on social services and agency services which will address their needs and make the appropriate referrals on behalf of the client.
  • Utilize case management skills, such as brokering, advocacy, monitoring and discharge planning to link clients and their families to resources and services.
  • Schedule and/or participate in Case Conference with clients/caregivers, service care planning and negotiate agreements.
  • Provide discharge planning to disenrolled clients to ensure client’s needs are met.
  • Ensure all services are documented per state regulations.
  • Travel to meet with current and potential clients, attend community resource meetings and conduct client case management sessions in the community as needed.
  • Required to attend mandatory training classes in accordance with Kintock training policies.
  • Perform other work related duties as assigned by the Director.
