Job summary

EP!C seeks a Program Coordinator to cultivate and expand the agency referral base of contractual Host Home Providers, overseeing relationships with community organizations and maintaining an active waiting list. You will recruit new contracted Host Home providers and ensure they meet the requirements set by EP!C, DHS, OSFM, and other guidelines.

Responsibilities include overseeing department development, mandated reporting, HIPAA compliance, onboarding, training, and coordinating with Marketing

Qualifications

  • High School Diploma or GED required.
  • Bachelor’s degree or equivalent (preferred).
  • Knowledge of general principles of recruiting.
  • Possess a valid driver’s license, automobile insurance, and reliable transportation.
  • At least one year of experience working with individuals with disabilities preferred
  • Strong knowledge of Illinois CILA regulations, Medicaid Waiver programs, and person-centered planning

Responsibilities

  • Support the agency’s mission, vision, values, and strategic plan philosophy.
  • Manage all responsibilities based on the agency's mission, using individual choice, and all appropriate policies, procedures, and regulations.
  • Conduct oneself in a way that reflects the highest standards of professional courtesy and ethical practices to people served, staff, guardians, families, and all outside contacts.
  • As a mandated reporter, ensure all allegations of abuse and neglect are reported as required.
  • Ensure compliance with all HIPAA and confidentiality requirements.
  • Complete all mandatory documentation accurately, honestly, and on time.
  • Recruit and ready new contracted Host Home Providers.
  • Verify qualifications and compare to EP!C mission and expectations.

  • Ensure that the Host Home Program increases its revenue base in accordance with the Business Plan approved by the CEO and the EP!C Board of Directors.
  • Coordinating with Marketing Department to develop and maintain an inventory of approved marketing materials.

Duties / Responsibilities

Department/Program Development and Coordination (Delivery of Quality Service)
  • As a mandated reporter, ensure all allegations of abuse and neglect are reported as required.
  • Ensure compliance with all HIPAA and confidentiality requirements.
  • Complete all mandatory documentation accurately, honestly, and on time.
  • Need to complete all required training within 120 days of initial employment and stay in compliance with all the required annual training refreshers.
  • Recruit and ready new contracted Host Home Providers:
Evaluate Candidates To Determine Eligibility
  • Verify qualifications and compare to EP!C mission and expectations
