Job summary

CareRite Centers, LLC seeks a Recreation Therapist to join our Community Based Adult Day Health Care Center. You will plan and conduct group and individual recreation programs to support residents' well-being.

You will document attendance and participation, facilitate independent activities, and assist residents to and from programs. You will participate in the care plan team to determine therapeutic objectives and create patient-specific care plans.

Qualifications

• Planning and conducting group and individual recreation programs as assigned by the Director of Recreation
• Documenting program attendance and participation
• Facilitate independent activities for residents
• Assist residents to and from recreation programs
• All other tasks as assigned by the Director of Recreation
• Work as part of the care plan team in determining therapeutic objectives and goals.
• Create patient specific care plans
• Other duties as required
