You will document attendance, participate in care plan meetings and collaborate with staff to support individualized care plans and well-being across the community.

Qualifications

• High school diploma or equivalent required.
• Must be able to read, write, speak and understand English to ensure safety and well-being of patients and visitors.
• Effective communication and strong organizational skills are essential.

Responsibilities

• Plan, organize and implement engagement activities meeting physical, cognitive, social and spiritual needs of patients/residents.
• Document attendance and participation levels for recreation programs.
• Attend care plan meetings as needed.
