Job summary

Wellness Ranch, part of Quadrant Health Group, seeks a Full Time Aftercare Coordinator in Franklin, KY to support clients after discharge. You will coordinate housing, employment, education, and recovery services, working with clinical staff to ensure continuity of care for ASAM Levels 3.5 and 3.7-WM clients.

Responsibilities include developing discharge plans, coordinating referrals, and assisting clients with access to needed resources.

Qualifications

• Knowledge of community resources related to substance use disorder treatment and recovery support.
• Ability to navigate social service systems.
• Strong organizational and communication skills.
• Ability to maintain professional boundaries with clients.
• Ability to work effectively within a multidisciplinary treatment team.

Responsibilities

• Coordinate housing, employment, education, legal matters, and recovery supports.
• Refer clients to community providers and recovery resources.
• Help clients schedule appointments and access services.
• Develop discharge and continuing care plans in collaboration with the clinical team.
• Coordinate referrals to outpatient treatment and sober living programs.
• Assist with obtaining documentation needed for a successful transition.
• Maintain communication with referral sources and community partners.
