Job summary

CHR Career Opportunities is seeking a Recovery Support Specialist for Rural Pathways in Danielson, CT. This full-time, on-site role involves conducting assessments, developing person-centered plans, and providing motivational and psycho-educational recovery support.

Qualifications include a required Recovery Support Certification or within-one-year attainment, 2+ years experience with identified populations, and a valid driver’s license. Competitive benefits and ongoing training are offered.

Qualifications

• Valid Recovery Support Certification required or must obtain within one year of hire.
• Valid Driver's License required, use of personal vehicle for position.
• Experience with mental health or substance use recovery populations.

Responsibilities

• Conducts Clinical and Risk Assessments, LOCUS, Co-occurring Screens and Functional Assessments.
• Collaborates to create individualized, person-centered treatment plans and documents client progress.
• Use Motivational Interventions to assist clients in meeting goals and moving along their paths to recovery.
• Provide psycho-educational rehabilitation services group and individual for recovery from psychiatric and substance abuse disorders.
• Support individuals with linking and engaging with natural or community supports.
• Develop crisis plans (WRAP) and respond to crisis situations as needed.
• Provide skills teaching for housing, transportation, nutrition, family communication, and safe community actions.
• Intensive skill building for self-management, self-advocacy, health and well-being.
• Participate in family education and support activities.
• Assist clients to identify and combat negative self-talk and model hope through lived recovery experience.
• Maintain documentation in compliance with agency standards.
• Participate in staff meetings, supervision, trainings and other meetings as assigned.
