The Forward Trust in Ashford is seeking a Recovery Worker to join our team on a full-time basis, 35 hours per week. You will support adults 18+ facing substance issues as part of an integrated healthcare team, providing person-centred care and practical help to move clients toward lasting change.

Responsibilities include managing caseloads, completing assessments and care plans, delivering 1:1 and group sessions, and helping clients access housing, benefits and healthcare.

Qualifications

• Experience supporting adults 18+ with drug and/or alcohol issues.
• Experience in a multidisciplinary healthcare setting.
• Ability to provide person-centred support and tailored interventions.

Responsabilités

• Managing your own caseload and supporting clients throughout their recovery journey
• Completing assessments, risk assessments and care plans
• Delivering structured 1:1 interventions and group sessions
• Providing harm reduction advice, guidance and practical support
• Helping clients access housing, benefits, employment, training, healthcare and community services
• Working closely with healthcare professionals, probation and partner agencies
• Reviewing care plans and supporting positive outcomes
