Job summary

Primary duties include coordinating care, making referrals, following discharge plans, and collaborating with a multidisciplinary team. The position offers a stable schedule and mileage reimbursement as applicable.

Qualifications

• Must have a High School Diploma or GED.
• 2 years of recovery time from a drug and alcohol addiction or mental illness.
• Peer Recovery Specialist Training certification from the Tennessee Department of Mental Health and Substance Abuse Services.

Responsibilities

• Work with FEPI consumers to reduce and manage psychiatric symptoms and facilitate their return to a less restrictive environment.
• Provide Consumer care within the parameters of the Coordinated Specialty Care (CSC) model.
• Provide linkage and referrals to supports, services, and groups.
• Provide follow-up contact with consumers upon discharge to ensure discharge plan is working and assist consumer if discharge plan is not working to develop alternatives.
• Monitor consumer’s behavior, motivation level and plan implementation, and safety in the community.
• Support consumers in understanding their psychiatric diagnosis, treatment options and medications.
• Support consumers and team members with discharge and treatment planning.
• Treat consumers and co-workers with respect.
• Documentation as required by agency.
• Develop and maintain community and inter-departmental relations through prompt follow-up.
• Customer service with internal and external customers.
• Communicate effectively with staff, supervisors, and consumers.
• Attend staff meetings, trainings, and supervision as required.
• Discuss precipitating events and possible alternatives with each consumer.
• Advocate for consumer needs.
