Job summary

Louisville Metro Government seeks a dedicated social service worker to implement and run peer mentoring and support programs for homeless individuals and those in permanent supportive housing. You will help clients develop recovery plans, provide illness-management education, and coordinate services as an extension of case management.

You will also train peer mentors, support street outreach, and conduct home visits to reinforce life skills and recovery while assessing client needs and matching

Qualifications

• High school diploma or equivalent.
• Must complete Kentucky Peer Support Specialist training and certification within one year of employment and maintain certification.
• Equivalent combination of training and experience may be substituted by Civil Service.

Responsibilities

• Assist clients in developing recovery plans.
• Provide education focused on recovery, illness management, and peer support.
• Provide case management, resourcing, and service coordination as an extension of case management.
• Facilitate small peer support groups, events, and programs.
• Support clients in strengthening life skills and recovery.
• Assist in training peer mentors on peer support services.
• accompany street outreach teams to provide peer support.
• Conduct home visits to provide life skills training and peer support; assess client needs.
• Help determine matches between peer mentors and recipients.
• Perform related work as required.
