CN Guidance & Counseling Services, Inc. is seeking a Peer Specialist to support residents in OMH Housing. You will serve as a health coach and role model, assisting clients with engagement, recovery and community living goals.

The role requires personal experience with disability or psychosocial challenges, a NY driver’s license, and a commitment to recovery-oriented care within a mobile residential team.

Formación

• High School Diploma or GED is required.
• Personal experience living with a disability or social/emotional challenge and willingness to use that experience to support recovery.
• Current New York State Driver License and clean DMV record are required.

Responsabilidades

• Provide outreach to clients to facilitate engagement in services and community living.
• Offer health coaching and recovery support focusing on self-advocacy, wellness, medication adherence and daily living activities.
• Coordinate transportation and travel training for clients as needed.
• Accompany clients to appointments and support engagement and follow-through.
• Provide concrete services such as benefits enrollment as required by the program.
