Job summary

Mental Health Cooperative of Nashville, TN seeks a Peer Support Specialist, FEPI to assist consumers with recovery and connect them to natural and formal supports within the CSC model.

The role involves engaging with individuals to reduce psychiatric symptoms, provide referrals, and support discharge planning while maintaining documentation and collaborative communication with the team.

Qualifications

• High School Diploma or GED required
• 2 years of recovery from drug/alcohol addiction or mental illness
• Peer Recovery Specialist Training certification from Tennessee Department of Mental Health and Substance Abuse Services

Responsibilities

• Work with FEPI consumers to reduce and manage psychiatric symptoms and facilitate return to a less restrictive environment
• Provide linkage and referrals to supports, services, and groups
• Conduct follow-ups post-discharge to ensure discharge plan effectiveness and develop alternatives when needed
• Monitor behavior, motivation, and safety in the community
• Support understanding of diagnosis, treatment options, and medications
• Assist with discharge planning and community linkages
• Maintain timely documentation as required by the agency
• Engage in team communications and staff meetings
