Job summary

Wayspring in Rockford, IL is seeking a Case Manager to expand access to care for individuals facing substance use disorder and related barriers. You’ll be a frontline navigator, meeting people where they are, building trust, and guiding members into Wayspring’s clinical services with compassionate engagement.

The role requires regular daytime travel in the greater Rockford area, mileage reimbursement, and the ability to enroll members in community and facility-based programs.

Qualifications

• Minimum of three (3) years of experience in a community outreach role or similar.
• Ability to communicate effectively and persuasively with diverse audiences.
• Exceptional verbal de-escalation skills and crisis management capabilities.
• Experience with CRM platforms is preferred.

Responsibilities

• Facilitates and secures enrollments from members in the community or in residential treatment centers.
• Assesses member needs quickly and triages accordingly.
• Engages members and provides an educational overview of Services offered.
• Obtains consent and paperwork for services from members to participate.
• Arranges first appointments with clinical services for members.
• Maintains frequent touchpoints to assess ongoing needs and determine visit frequency.
• Develops strategies to facilitate member contact and engagement.
