Job summary

Bay Cove Human Services in Greater Boston seeks an Integrated Team Engagement Partner II to support adults with mental health challenges. You will work within the ACCS Integrated Team delivering interventions and treatment planning guided by evidence-based practices, with a focus on helping persons reach independence and improved quality of life.

Qualifications

• Bachelor's degree in psychology, social work, or related field.
• High School diploma with 3+ years direct care experience acceptable.
• DPH medication administration certification within 3 months.
• CPR and First Aid certification within 3 months.
• Valid driver’s license preferred.
• Bilingual candidates encouraged; pay differential may apply.

Responsibilities

• Incorporate evidence-based practices into engagement strategies and treatment planning.
• Participate on the Engagement Team for assigned persons per supervisor.
• Use recovery-oriented language in all communications.
• Teach daily living skills and support community integration.
• Coordinate with care entities to ensure access to medical and peer modalities.
• Accompany clients to appointments and activities when needed.
• Support housing coordination to meet licensing requirements.
• Administer medications per MAP standards and train clients.
• Perform other duties as assigned by supervisor.
