Qualifications

• High School Diploma or GED required.
• 1 to 2 years related work experience.
• Ability to work with diverse populations.
• Knowledge of substance abuse services.
• Ability to build trust and form impactful relationships.
• Proficient with Microsoft Office (Word, Outlook, Excel).
• Strong collaboration skills and ability to work independently or in a team.
• Comfortable in a fast-paced environment with deadlines; energetic and proactive.
• Excellent communication and interpersonal skills.

• Must have a valid driver’s license and a vehicle with liability insurance.

Responsibilities

• Supports client engagement and ongoing connection throughout treatment, including outreach.
• Develops therapeutic relationships with clients and families to support recovery goals.
• Participates in meetings with new referrals to explain program structure and role.
• Provides community- and home-based support including school, employment, and wellness activities.
• Co-facilitates peer-led and recovery-focused groups (IPS, SRAP, WHAM, among others).
• Shared lived experience to model recovery and promote hope.
• Connects clients with supports and peer-run services for long-term stability.
• Assists with community outreach to promote First Episode Psychosis services.
• Maintains documentation in compliance with HIPAA and agency standards.
• Participates in NAVIGATE/OnTrackNY alignment training and supervision.
• Performs other duties or special projects as assigned.
