Job summary

South Shore Association for Independent Living seeks a dedicated professional to support consumers in maintaining sobriety and stability while linking them to treatment services within a community setting.

You will coordinate with Clinical Coordinator, Supervisor, and Care Managers, visit clients regularly, teach independent living skills, and develop recovery plans. Evening/weekend availability may be required.

Qualifications

• Bachelor's degree in Human Services (BSW preferred).
• 3+ years experience with target population.
• Organizational, Communication, Critical Thinking, And Judgment Skills Required.

Responsibilities

• Encourages and supports consumers with sobriety, stability, and treatment linkages.
• Coordinates services with Clinical Coordinator, Supervisor, and Care Managers.
• Visits consumers at least twice monthly or as needed.
• Teaches independent living skills to sustain housing and prevent homelessness.
• Develops Individualized Recovery Plans and monitors progress on goals.
• Ensures fire safety equipment is present and educates residents on use.
• Teaches emergency protocols (medical, psychiatric, fire, food, etc.).
• Acts as liaison with landlords, teams, and mandated treatment providers.
• Completes timely documentation and attends required meetings.
