Qualifications

• Bachelor’s degree in Human Services field with 2+ years full-time experience with population served.
• Bachelor’s degree in non-Human Services field with 4+ years full-time experience with population served.
• Master’s Degree or Higher with 1+ years full-time experience with population served.

Responsibilities

• Provides support to individuals with mental health and/or substance use issues in home, school, workplace and community settings.
• Focus is on assisting individuals with living independently through strength-based interventions promoting recovery and enhanced quality of life.
• Facilitating relationships and serving as a link between individuals served, the company, families/guardians, local agencies and the community.
• Performing case management functions for linking and arranging services and referrals.
• Working with clinical staff to maintain communication and expedite service implementation.
